An electronic change request form is submitted to the change control board. The change control board can approve the change, request further study, or disapprove the change. If the change is approved or disapproved, then they report the decision. If they request further study, then an evaluation team is formed. The evaluation team takes the following actions:

• Evaluates the request with the requestor
• Closes outstanding actions
• Reports findings to the change control board

The change control board then puts the request through the review process again.
